No insurance, no problem! We are committed to providing excellent dental care in a comfortable environment, at an affordable price. Our dental membership plan provides a budget-friendly and flexible alternative to expensive dental insurance so you can receive quality dental care at an affordable price.
What are the benefits of the membership program?

What does the membership include?
- 2 exams
- 2 “healthy mouth” dental cleanings OR 2 periodontal maintenance
- All x-rays deemed necessary by dentist (bitewing, periapical, and panoramic radiographs)
- 2 applications of fluoride varnish
- 50% discount on CBCT x-rays, custom whitening trays, and emergency exams
- 10% fee reduction on all other procedures completed in the office including but not limited to: Fillings, Crowns and Bridges, Veneers, Dentures (complete and partial), Scaling and Root Planing, Extractions, Root Canals, Implant Placement, Night guards
What is the cost?
- Adult cost (18+) – $320/year
- Child cost (3-17) – $260/year
- This fee must be paid in full on start date and no refunds may be given
- Benefits begin on date of payment and continue for 12 months
- Renewal will take effect the day after the previous membership ends
Limitations and Exclusions
- If a patient becomes covered by a traditional dental plan, their membership plan becomes null and void with no refund of fees
- Any service referred out, including but not limited to: endodontics, periodontics, orthodontics, and oral surgery
- CareCredit may be used to pay for treatment, but all other discounts will be forfeited
- Services which, in the opinion of Dr. Sladky, are neither necessary nor recommended for the patient’s health

18+ years
$320/year
- 2 preventative care appointments each year with necessary x-rays, exams, and fluoride treatments.
- 10% discount on most restorative procedures including implant crowns
- 15% discount if treatment is paid in full with cash or check at the time of services
- 50% discount on emergency exams
Child Membership
3-17 years
$260/year
- 2 preventative care appointments each year with necessary x-rays, exams, and fluoride treatments.
- 10% discount on most restorative procedures including implant crowns.
