Do you accept new patients, and what should I bring to my first appointment?
Yes! We’re accepting new patients at our Glendale office. For your first visit, please bring completed new-patient forms, any recent X-rays you have, a valid photo ID, a list of medications or medical conditions, and your insurance card if you have one.

What types of dental services does Sladky Family Dental provide?
We offer a full range of services, including preventative care (cleanings, mouthguards, periodontal treatment), restorative care (fillings, crowns & bridges, implants, dentures, root canals, wisdom tooth extractions), cosmetic dentistry (veneers, Invisalign®, whitening), sedation dentistry, and emergency dentistry.

If I have a dental emergency, can I be seen the same day?
Yes. We reserve time in our schedule to accommodate dental emergencies, so you can be seen promptly and start receiving relief.

Which insurance plans do you accept, and what payment options are available if I don’t have insurance?
We accept most major dental insurance providers and file claims on your behalf. We do not accept state insurance programs such as Medicaid, BadgerCare, Molina or Forward Health. For patients without insurance, we offer an in-house dental membership plan and partner with financing via CareCredit®.

How often should I come in for cleanings and checkups?
For most patients, we recommend cleanings and exams every six months to maintain oral health and catch issues early. If you have higher risk factors (gum disease, many restorations, etc.), we may recommend more frequent visits.

What technology or comfort features does the office provide?
We use up-to-date dental technology—digital X-rays, intra-oral cameras, CBCT (3D) scanning, digital impressions—to enhance accuracy and comfort. We also offer sedation dentistry via nitrous oxide and oral conscious sedation to ensure a relaxed experience.

Where are you located, and what are your office hours?
We’re located at 500 W Silver Spring Dr., Suite K275, Glendale, WI 53217. Our hours are Monday 9:00 a.m.–5:30 p.m., Tuesday 9:00–5:30, Wednesday 9:00–4:30, Thursday 8:00–1:00, and we are closed Friday through Sunday.
